Transitions provides supports to young adults who have recently graduated or “aged-out” of the school system or are transitioning into a new program and/or employment or housing environment. Partnerships have been developed with local schools to provide person-centered transition supports.
Key focus areas can include employment skill building, decreasing levels of supervision and building of social skills. Transition services are expected to last for a period of approximately one year and with the duration being tailored to the needs of the individual.
Ages: Young adults 16+ (Age 16-20 is in partnership with school district)
